Health Claims must be submitted to our office within ninety days from the date the expense was incurred.

RMS BEREAVEMENT INSURANCE CLAIMS

There is a six-month waiting period after the policy is first effected. If a person dies from natural causes during the waiting period, a claim will not be paid. However, if a person dies from accident within the six months, a claim will be paid.

 

To file a claim, the policyholder’s estate or beneficiary, must present the original death certificate and proof of age showing name and date of birth of the deceased and the claimant in the form of: Identification card,
driver’s permit, passport or birth certificate. In addition, the Insured Member’s Certificate of Insurance
